Your claim needs to be accepted and you need to receive a claim number.

Fill in the form with your claim number, name, and contact information.

Once we have confirmed the authorization of your claim with the SAAQ, we will contact you back to book an evaluation.

Please bring with you to your first visit:
a) physiotherapy prescription
b) SAAQ claim number
c) Relevant medical information related to your injury
